because it takes on the size of the currently selected brush size.It is possible to customize the way the annotations are shown. Brush size can be set to thin, normal, or thick and an appropriate color can be picked.
To set the brush size for the annotation tool, take the following steps:
1. Click Size
in the Annotations group of the
Image Viewer toolbar. The size of the icon as it is displayed varies,
because it takes on the size of the currently selected brush size.
2. Select the appropriate size from the menu. The brush is available in the following sizes:

To set the color for the brush:
1. Click Color 
in the Annotations group of the Image
Viewer toolbar. The color of the icon as it is displayed varies,
because it takes on the currently selected color.
2. Choose a color.
3. For additional colors, click More Colors. Select one of the basic colors or compose a custom color. Then click OK.
4. The selected color will now be used to make annotations. Additionally, it will be included in the Recently used section of the color panel.
